"Ex" is a preposition that means "of" or "from", but usually in the sense
of "out of".
"Cor" does indeed mean "heart", but you want the ablative case since the
preposition "ex" takes the ablative.
So, I'd say "ex corde semper." Actually, I'd prefer "semper ex corde",
but the word order is strictly up to you (it's not
my chest).
