I am looking for the text of a Medieval prayer which I have in English but want in Latin. If someone will translate the first line, I believe I can find the whole text, in Latin, by Googling those words. Here goes:
O Mother of Perpetual Help, grant that I may ever invoke thy most powerful name...

Here's how I would render it.
O Mater Auxilii Perpetualis, concede ut nomen tuum potentissimum umquam invocem.
