Does "even to those in their grip" refer to "even those desires that are in their control are pathological as well"?
Context:
According to compatibilists, if a man wants to commit murder, and does so because of this desire, his actions attest to his freedom of will. From both a moral and a scientific perspective, this seems deliberately obtuse. People have many competing desires-and some desires appear pathological (that is, undesirable) even to those in their grip.

Some desires appear pathological even to those people who are having the desires (and who are powerless to resist having them).

I'm sorry it was still offering no help.
My understanding seems to protest that the people who are powerless to resist having pathological naturally suffer such pathological thoughts/desires. Isn't it useless? Supposed one is powerless to resist illness, one of course gets ill often. To say illness even appears to powerless people is a tautology.

The control is held by the pathological desires over the people who have these desires. The pathology of these desires is perceivable even to these people.
