I had a 70 years old patient complaining of cough and weakness for 3 days. She was ytpe2 DM and esansial HT and on medication. Her arrival values
BP : 110/60 hr: 120 spo2: 96 glucose : 595
troponin (-) WBC: 14000 CRP: 133
urea creatinim electrolits normal
ECG - normal norologic examination - normal chest xray - normal
urine strip - normal ENT examination - normal
my treatmenr in ED was;
regular insuline 16U sc
seftriacson 2gr iv
ornidazole 1gr iv
diclofenac im
NS 2lt iv in 3 hours
after 3 hours her parameters was ;
BP: 80/60 HR: 75 SPO2: 96 glucose: 225
she was not hyperhpneic and other examinations were normal as her admission. There is not bloodgas in my department so i couldnt take it.
i monitored her a hour more. and BP decreased 60/40 HR:78 GCS:15
i cant understand the reason of hypotension. is there any idea
