They don't. They're just convenient fictions.
that´s a to easy of an answer...if concepts are "convenient fictions", who is to say what is "real" instead, once concepts are all you are left with ???
In the absence of anything else the "real" is what you are left with !
I'm standing by it, tho. There's a significant difference between sensory input and abstractions drawn from them. The abstractions are one step removed from the phenomena. The phenomena are "it". When the mind conceptualized and translates into language, the mind is manipulating the data to make it useful. Great for survival, mind you, but it tends to deceive one into believing that the abstract concepts are the real and that perceptions/phenomena are fallible. 'Tis the other way 'round, as far as I can tell. So far.
Of course, I may be wrong.
Wouldn't be the first time.
Today.
