This is a relative's birth certificate and I was wondering if anyone could make out little parts of it for me. I would be very grateful!

Nothing complicated, will give you the translation later on...
That's really hard to read. I tried to make the contrast stronger, which helped a little. But, when I blew it up it got pixilated - hard to read. I know Francis will have good luck with it.
