Could you explain this sentence"she is ill at ease ." Is she really ill ?

"Ill at ease" means that she is feeling uncomfortable. It is more of an emotional discomfort, than a physical one.

"The first time that she met her boyfriend's parents, she was not sure that they would like her, so she felt very ill at ease".
