It means to be aware of something.

It might be a warning:

"Watch out for taxi drivers who will try to cheat you on the cost of the trip."

Or it might be simply a means of identifying someone or something.

"Watch out for someone wearing a bright green jacket--that is how you will be able to find me."

Means to be "on the look out"... Do you know anything about American History? Have you read about the American Revolution? During this time of turmoil, the Americans were "on the look out" for attacking Indians and of course, the BRITISH!
