Gloria: My mother-in-law is always criticizing me. She doesn't like my cooking, or the way I take care of the children. What advice do you have for me?
Dr. Burk: Gloria, I have a couple of ideas, but first, how big is your house?
Gloria: We're lucky. It's quite big.
Dr. Burk: well then, here's a suggestion. You could put in a little apartment for your mother-in-law. You just need a small kitchen, bathroom, and a bedroom.

Dr. Burk is telling Gloria that since she has a big house, she could convert part of the house into a little apartment for her mother in law. It might have a separate entrance into the house.
BTW, sometimes when you see a house advertised, it will say something about a "MIL suite" or "MIL house" That would be either that type of convertion done to the house, or a conversion of a garage or other separate building on the property to living quarters...AKA guest house.
Nowadays, people will rent out their MIL suite or house to someone.
