The answer to "who holds a fief?" could be "noble."

I wonder if you don't have a misspelling there. A fief is a feudal estate, which is "held" at the pleasure of a feudal lord. So, for example, an Earl (which is the equivalent of the continental rank of Count) holds the feudal title to a county. He would parcel out the land (and its attendant income) to a series of lower members of the nobility (barons, usually) who would "hold" the land which was in the gift of the Earl. They would then be said to be fiefholders. Check your spelling.

As I am only a Lord, I am restricted to being a mere fourfholder.

...and so it works down through the ranks.

There is a rank for professional/medical people, believe it or not.

A Dentist, for instance, is a toofholder.
