If Lewis is correct and if the "imported" insurgents come from the group he identified, then it appears that we, together with moderate Muslims, must work to remove all such fanatics from the face of the earth.

Not when Saudi-Arabian fanatics can cause 9/11's.
If recent history is any guide, the "moderate muslims" aren't going to be much help[.
Islam is not like other religions and cannot be treated as if it were. Ultimately, western nations including us will have three or four choices in dealing with Islam:
- Submit, and allow your nation to come under islammic law.
- Ban Islam altogether.
- Establish some official version of it which will be tolerated, and proscribe all others.
I'm not sure I know what a fourth or fifth choice would be.
