natasha to be obsevred helping a old lady to prepare to eat and drink and to consume diet and fluid. this lady has severe swallowing problems
i need to do it with
how
when
who
what
why
can someone please help me get on the right lines
asap.

When: each time the old lady eats and/or drinks

Who: Natasha, the old lady and the observer

What: swallowing problems

Why: to help the old lady

I was observed by boss Matron In helping an old woman with severe swallowing problems. To prepare to eat and drink at meal time and to consume her diet and fluid.

is that ok what more can i put

Well, your 2nd sentence is incomplete, but it can be changed:
