Which sentence is correct to describe the situation?

My leg was not amputated.
My leg is not amputated.
My leg has not been amputated.
My leg had not been amputated.

Since all the events occurred in the past and are complete, use the simple past tense (met with, broke, was not). The first two verbs are there for you to see.
