It means the same thing as "Are you high?"

Essentially it means that the person's thinking is faulty; that they're being irrational.

The expression comes from drug use. People who are under the influence of a mind-altering drug such as LSD are said to be on a "trip".

It is similar to asking; "Are you on drugs?"
