Which of the following sentences are correct?

Vendor Reference and Vendor Contact is required.
Or
Vendor Reference and Vendor Contact are required.

Vendor Reference and Vendor Contact are required.

But if they aren't specific brand names, you can simplify the sentence to read as:
"Vendor reference and contact are required."

Capitals shouldn't be used if the nouns in question aren't proper product names.
