Yes

Quote:
2. I have___many fishes in that stream. Caught?

Yes

3. Yesterday he___late to school. came?

Yes, but the word order is wrong. "Came" and "to school" should not be separated.

Quote:
4. I have__my pencil. broke or broken?

Broken (the past participle).

Quote:
5. The flag was ____ at dawn. Raised?

Yes

teacher's. One person possessive.

Is the word weeks correct or should I use week's or weeks'?

two-weeks notice, or two-week notice. By the letter of teh law the adjective should not be plural so when it's "two-weeks" it's really saying "two weeks of notice" and not a "two-week notice".

This sentence still feels awkward. I would say "I caught many fish from that stream." I'm not looking at it grammatically though, it just felt awkward the way you worded it.
