Her agitated hands wrung anxious folds through the unseated shoulder of her sari.

She was anxious, and agitated. She wrung her hands (twisting them together), and wrung part of her sari too, folding it or creasing it. Evidently the sari was not sitting snugly on her shoulders, perhaps because she had dislodged it with her hand-wringing.
