For example, George Bunglestein may be known to his friends as Hoover.

Sorry what I left out was the first question and that asked for the official title of the person.

My doctor's name is Dr. Karel Jill Ortiz. She goes by, and is professionally known by the name of Jill rather than Karel.

So, in the format you suggested, she'd probably complete it as follows:

"Honoree First Name: Karel
Honoree Last Name: Ortiz
Honoree Name:" Jill Ortiz or Karel "Jill" Ortiz
