what does " for the name of your nearest store" mean?

Not every men's store sells Hathaway shirts. If a person wanted a Hathaway shirt, he would phone the number, and the person answering (who works for Hathaway) would ask the caller for his address. The employee of Hathaway would tell the person which store is nearest to the customer's home, so that he could buy the shirt in a convenient place.

That ad must be VERY old. A modern ad would have a website that would offer the same information. Also, no Hathaway shirt would sell for the prices that are in the example that you have given.

Another clue that it is very old is that the telephone number uses an alpha-numeric exchange, and doesn't give an area code.
