Format of medical history file

I want to open a medical history file for each indivisual member of my family. So that when ever any family member needs to visit doctor due to any disease, I can put his diagnosis, prescription, and medical reports in the indivisual's file. This file will be helpful for doctor on next visit, to understand the medical history of the patient.
Now my question is, what should be the format of this medical history file?
