Does "desparately" mean the same thing in both contexts? that is, "extremely"?
When one is desperate, one will just about do anything. It means it is not something they might want to do but they have very few or no other option.

It means intensely, with great anxiety. In the second line, it contrasts with and contradicts "casually", saying that while they are trying to appear casual, it is obvious that it is anything but.
