Perkowski is a Polish name.

Some Perkowski were famous Poles, like Piotr, a well known composer..

That was one of my uncles's last name.
100% Polish.

The question, with its either-or expectation, is a bit artificial; rather like "Is Jones a Welsh name or an English one?". It is very clear that there are very many Perlowskis in Poland, but I doubt very much that there are no Perlowskis at all in Russia. I don't think that Poland can lay claim to exclusive ownership of the Perlowski name.

Everybody has doubts about this or that.

However, given the endemic use of Perkowski in Poland compared with the few examples found in Russia, I've no doubt about this one.
