ending just doesn't feel right

The dentist first makes a mould of his patient's teeth from which he makes a model of the teeth how to correct any problems.


ending of this sentence ''how to correct any problems'' doesn't look right to me but I can't put in words exactly whats wrong with it.
Any help will be appreciated.

Doesn't look right to me, either. It's almost as if a word is missing.

How about;

The dentist first makes a mould of his patient's teeth from which he makes a model of the teeth to indicate how to correct any problems.
