What date and time did you last called your mistress?

Is the sentence correct?

No, it is not correct.

You have a past tense of a verb, divided by the subject. "What date and time did you last call your mistress?"--would be correct. You might understand it better by uniting the subject and verb: "What date and time did you call your mistress last?"

The verb is "did call." If you use "did called," you have combined two simple past forms of the verbs "to do" and "to call." That's a no-no.
