Little that he realises that he is being mean?

I believe 'that' is not the correct word. Instead 'did' should be used Am I correct?

"Little that" is totally wrong.

You could use "does"(present) or "did" (past).

Little did(past) he realise[/u]

Little does(present) he realise[/u]

Or

He little realises
