They are all rather poor, I am afraid. It is not clear what they mean.
(1) means, I suppose, "I can't remember the period of time time when I still was in the habit of eating breakfast (every day) (implication: which I am not now)". I.e. nowadays he gets out of bed and eats nothing until lunchtime. It is contadictory, since the speaker is talking about the period of time which he claims not to remember.
(2) might mean "I cannot remember what time it was (i.e. eight o'clock, eight-thirty, nine o'clock etc) when I last ate breakfast"
(3) could possibly mean "I can't remember when I last ate breakfast". Again, the implication is that he doesn't bother to eat breakfast.
A "last breakfast" is the meal a condemned prisoner has shortly before being executed! (If he has any appetite.)
Maybe what is meant is "I can't remember when (i.e. what time) I had breakfast today"?
If that is so, then the sentence could be "I can't remember when I had breakfast". ("Today" is understood.)
