Hi i'm a 22 years old and i am really concerned for my 19 year old brother. He has extremely low self esteem for somone of his age group. I am worrying if it is effecting his social and work life. I am quite sure that has to do with his body image. He is constantly telling me that he is incredibly ugly. I don't know what to say to him because i personally don't think hes naturally ugly but he is pretty much convinced he is hideous. Based purely on looks, would you say he is ugly?? I'm not going to tell him this as I am currently looking for ways to help him. I just want to define the problem is.

Does your brother go to school? Does he have a job? Does he have a social life? What does he do with his time?
Goodness knows what he means with "I'm ugly". Obviously this is not an objective opinion, but it seems to be a deeply held belief. If your brother spends all of his time sitting in front of electronic gadgets muttering, "I'm ugly," he should talk to a counselor who can help him see himself and the world more clearly.
Does he use his "ugliness" as a reason for a restricted life?

No, he is not ugly.
Maybe just a more flattering haircut and a bit of a wardrobe update would be the only surface things I'd suggest. And a smile.
But as Noddy said, it sounds like an underlying problem.
No he's not ugly. He's actually kinda cute.
He is ugly and should focus on his personality.
