Are the words correctly used?

As Tom was being led away by the police, his mother was crying her heart out. She kept on reaching out for him, only to be held back by the police who were at the crime scene.

1. Are there any errors in the sentences?
2. Are 'heart out' and 'reaching out for' correctly used?
3. Is 'crime scene' correct or should it be 'scene of crime'?

2. Yes

3. Either would do. It's "the scene of the crime", though
