8/9 should be read as '8 out of 9', '8 over 9' or '8 upon 9'?

8/9 can be read as any of the 3 you listed in addition to 8 divided by 9. When speaking, I would say eight ninths. It may be different depending on where you live.
I've never heard of "8 upon 9"... but the other two are correct.

I never heard of 8 upon 9 either. Probably another British/American thing.

Australian is close to british English.
8 upon 9 is not something I am familiar with.

8/9 could also be read as "eight or nine".
