a. is okay although the context is probably involved. For some reason he continued to monitor them after its failure
b. Can't be seen to make sense
c. says that the recording of data was somehow the very cause of failure
d. is perfectly okay, fits the usu circumstances, a failure in spite of his diligence

That's been my training style in bookkeeping, where I ask the question and they respond in many different ways. When the result is the same, the entry can be somewhat different. I always try to look at what's called "materiality."
If it doesn't impact the results materially, I approve of how they enter the numbers.
I have trained management staff at Florsheim and at smaller companies with less than 100 employees.

I've always said I've been lucky to have been at the right place at the right time. I still believe that to be true.
Before I even graduated from college with a degree in Accounting, I had four job offers. I took the one with Florsheim Shoes, because it meant I would be recognized for my performance rather than my scholarship, and I won't be competing with all those genius' I went to college with. Going to work for an accounting firm meant just that.
All turned out very well; right place at the right time.
Also, the competition at most accounting firms meant working 24/7. Many wives didn't appreciate that, and divorced. I also knew that working 24/7 at Florsheim meant better performance. Huge difference.
