She thought Joan the most beautiful of babies and she thought of Joan always as her baby, and Joan seemed to Millie to feel (which/that) Millie was her whole world, too.
So that was the question I had in a test. Does which and that both make sense? And can you explain why you chose your answer?

No, they don't mean the same thing in this context, not at all. Which refers to something from a set of things already mentioned. Joan's relationship is only with Millie.
