Is "Libum vel nex?" a suitable Latin translation for the phrase " cake or death?"

Yes, but . . .

Libum actually refers to a flat cake something like what we would call a
pancake. I looked for a general word for cake such as we know it, but
without much success. All the words I found denoted specific types of
cake. The closest word for cake in general seems to be placenta.
(Yeah, I know.) So libum is not a bad way to go.

Vel expresses a weak disjunction. Use aut for a strong disjunction such
as this.

Nex means violent death. A more general term for death is mors.
