death panels would be a good start but then we would still need to terminate all insurance/health care programs for everyone on their 60th birthday (maintaining adequate hospice care), this would apply to all private and/or government programs including medicare/medicaid. All long term/terminal illnesses would be limited to 90 days care and treatment. VA health care for post service related disabilites would be limited to prosthesis replacements (legs/arms etc) for 5 years. mental health/psychological illness would not be eligible for any treatment but psycho-pharmacological drugs would be made available at cost without prescription. dental/vision services would be limited to white canes for the blind and gerbers baby food for the dentally impaired.
while no health care insurance would be available outside the above restrictions, health care services would be available to those who could afford cash payment.

Stop Medicare (which starts at age 65) at age 60. Creative, confusing, and very cost effective.
If anybody is interested in reducing health care costs they have become marginalised in recent years and are politically a dead duck.
