Does anyone know a website I can find honest reviews for senior service providers such as assisted living facilities? I found one site called caregrade.com, but there were not that many reviews for my area.

I am in US. Looking for NJ or NY areas
As far as I know there aren't reliable regional lists of Nursing Homes. Remember, ownership and conditions in Nursing Homes can change overnight. Have you checked with your local Agency for Aging? They might be able to suggest (rather than recommend) places for you to investigate.
You can find Nursing Homes in your immediate area through the Yellow Pages, but I'd recommend vetting them personally.
