Does anyone know why I can't masturbate?

Im male 20 years old I feel like I'm bleeding internally and like I'm drowning, And heart beats too fast and the good chemical is too intense. Its like a sharp feeling of penetration into my neck or muscles, it doesn't hurt, but it feel like it's clogged, or maybe like bulging in my blood vessel. It makes me feel like I'm going to faint.
