Can heated milk that separated be recombined?

I guess I should also ask what I might have done wrong while making this delicious soup with fresh 2% milk (and butter/margarine, olive oil, a little evaporated milk,veggies). But at the moment I'd really love to learn a trick to get my curdled-looking soup back to smooth.
