Siemens make 25kva inverter in rail uses 1200v IGBT why failed?

In 25kva inverter, input 110v dc o/p 415vac 3phase IGBT failed some time, so in which cause the the the IGBT failed?can short ckt in out-side of the load cause d failure in the IGBT ? ?
