From February to July 2003, Toronto (population size 2,000,000) experienced the largest outbreak of the severe acute respiratory syndrome (SARS) outside of Asia. A total of 225 patients met the case definition of SARS, 38 died and 23,103 secondary contacts were identified for quarantine. What is the attack rate in this epidemic (per 1,000 population)?
