Well, you can do it in different ways, depending if it is a one time job or having a permanent solution.

I tried almost all the solutions above, they work fine..

we use Nuance's PDF Coverter for merging (and extracting) PDF's.

we rarely print drawings for submittals these days.

instead, we send PDF's and the client/owner does their own plotting...
