Peter groans at the tinny ring of the alarm clock. The bedsprings creak as he lifts himself off the thin mattress, though it could just as easily be his joints making the noise.
What does the sentence mean? Does it mean that the sound of Peter's joints creaking is as loud as the creaking of the bedsprings?
Yes, although human joints in the knees, back and hips would not make that creaking sound. It just FEELS as though they do.

Not likely as loud, but it may well be an exaggeration for emphasis, Nancy.
