Membership/ Registration form

This should be so simple but I've been struggling with it for an hour so I'm going to see if anyone can point me towards a shortcut.

I'm trying to make a registration form. Like name, address, etc. I can do a serviceable but crappy one in Word, I'd like for it to be somewhat decent-looking though. Word is giving me fits in that regard. Things aren't lining up right, I'm having a hard time getting the lines after things like "name" to work out, etc.

This will be a file on a website that people can then download, print out, fill out, and send in with their membership fee. (There will be a purely online version too, but some people get freaked out by those, plus people want to be able to print out 10 say and then hand them out to people who don't have computers.)

So, any pointers? I just want a nice, simple, somewhat professional-looking form. Templates would be especially nice.
