Trying to format a usb

Hello,

Im trying to format a my usb when i click format my usb shows the disk is write protected see the pic:

When i go from the cmd
i write attributes disk then shows the disk isn't write protected
http://prntscr.com/dkwtrx
Any help, how can i formate my disk?
