How do you know if a child who is old enough to know better is acting out for attention or has real problem? I.e. wetting the bed, sucking thumb, crying over lil things, highly defensive, not wanting to shower everyday?

By definition acting out implies that the child has inner stresses and emotions that they are not able to deal with.

That constellation of behaviours is concerning, especially if it is new. It implies the child is seriously distressed.

If it is ongoing, it implies a problem that needs dealing with. Asking constantly for negative attention can lead to serious problems. There are good strategies to help parents turn this around.
