If you understand PHP it should be simpler for you to write your own than to try to make sense of my code (un-commented).

It's just a simple PHP mail form (as I did not want it to be PM-based).

Eventually I will write it up as a MOD, but I'm really serious when I say it'd be easier for you to write your own than for me to find all the code snippets that make up this one and separate it from the proprietary code. It's just a mail form.

Ah and if it helps the relevant variables are passed through the URL and the form then mails to a mailing list.
