In the John Wayne film "Big Jake" Texas rangers in three 1909 cars with skinny spoked wheels try to head off kidnappers who have taken Wayne's grandson. While going down the road the camera angle was slightly above the three cars. There fairly obvious in the road were the camera trucks tire imprints for all to see. Oooops!

in 3 men and a little baby there is a scene where the propman left a cardboard cutout of ted danson in the background. that blooper turned into an urban legend about a ghost.
in the disney's animated movie, the rescuers, there is a topless women in the background of one scene. the video was recalled by disney in 1999.

There's a few in Arnie's Commando. Early on he goes into the shopping centre with the air-hostess and it's daylight, then leaves 10 minutes later after a fight and it's night-time. Later on he tracks one of the baddies out to a motel in a yellow sports car which had been in a crash of some sort. There are obviously at least 2 different cars used for the shot as the damage on the car not only changes in severity, but also the side of the car it was inflicted on. Still love the film though!
In Predator Carl Whatshisname's arm is clearly visible sticking out from his armoured jacket after it had been severed by the Predator's laser.
One thing I've never understood about just about all films that feature computers is the amount of flashing lights. Surely the point of a flashing light on a console would be to draw the attention of a technician to an event happening which wasn;t happening before. If one extra light (maybe the one that means the space-ship will explode or a reactor goes critical) starts flashing amongst 265 other ones - WHO'S GONNA NOTICE?
Spotted a mistake this morning in Clue. The chandelier falls to the ground with glass everywhere. In the next scene, the chandelier and all the broken glass are gone.
Did anybody notice the pillow, Rusel Crowes head was resting on, when he lay dying in the arena in Gladiator.
And the woman on a skateboard, sliding down the deck in Titanic.
