Proper Procedure for Signing Over Paternal Rights

I have a four year old daugher. Her actual father has had absolutely nothing to do with her, ever. His name is not even on her birth certificate. Now that I am getting married my fiance & I have agreed that he wants to adopt her. Her father has agreed to signing over his rights. What is the proper or legal procedure for him doing so in the state of Texas? Also, since he is not on her birth certificate, what does my fiance have to do to be able to adopt her?

You'll need a lawyer. If all parties are agreed the process shouldn't be overly complicated or overly expensive.
