Hey, are you really in Panorama City?
That looks fine to me. Request letters aren't like cover letters when you reply for a job, for example -- they don't have to convey anything in particular, just provide the needed information.
Only thing that really needs to change is, instead of "please mail this request to...", have "please mail the transcript records to..."
No particular reason it needs to be in the third person. Can just have "this letter is to request a copy of MY transcript records..." and then full name and address at the bottom.

Well that's certainly much better!
Part of my point is that if you have a lousy cover letter you won't get the job; if you have an awkward request for records that nonetheless is clear and contains the necessary information you'll still get the records.
Probably need to pay for it too, and have something in the letter about that. ("I have enclosed the $15 fee," or whatever.)
