I can understand how a key is the answer to the riddle - but what does it mean, the above sentence, in the context to the key?

Here's another similar to the President question: a boy and his father were involved in a horrific car accident and the father died. The boy was rushed to the hospital where the doctor took one look at him and cried out, "I cannot operate on him, he is my son!" Who was the doctor?
