Facebook: post video to my fan page as me (personal account)

You won't be able to post it simultaneously. You need to post to one and then share to the other. Start with the fan page as that will probably be easier. Pull down on the scroll wheel (usually in the upper right corner, but Facebook beta tests constantly, so it might be elsewhere) and select "post as me". Upload the video to your fan page. Then look for it in videos - it's probably one of your tabs.

Then share it to your own wall. Again, make sure you're posting as yourself.
