Have you noticed Crumb's attitude to women? Note his activity in the final panel. True, the girl does not appear to be resisting, but Crumb often made drawings like this. What does it say to you?

It says its a cartoon about society. Crumb comments on society and his own personal demons. Some times a cigar is just a cigar.
